What services does Choice Nursing Care provide under the NDIS?

We provide a range of services including Shared Independent Living (SIL), Specialist Disability Accommodation (SDA), Day Programs, Respite Care, Community Nursing, Daily Living Support, and High Intensity Support for participants with complex care needs.

Yes, Choice Nursing Care is a fully registered NDIS provider, approved to deliver a range of core and capacity-building supports, including high-intensity daily personal activities and complex nursing care.

You can contact us directly, or be referred through your Support Coordinator, Local Area Coordinator (LAC), or Plan Manager. We will work with you to assess your needs and develop a service agreement that matches your NDIS plan.

SIL is a support service provided in a shared home environment where participants receive daily assistance with personal care, cooking, cleaning, medication, and other daily tasks, while encouraging independence.

Participants with SIL funding in their NDIS plan and who require 24/7 or regular support to live independently in a shared setting are eligible.

A functional assessment (e.g. by an Occupational Therapist) and a Support Needs Assessment are required to apply for SIL funding. We can support you and your team in gathering the necessary documentation.

Yes, we aim to match participants with housemates who have compatible lifestyles, interests, and routines to promote a comfortable and supportive home environment.

SDA refers to specially designed housing for participants with extreme functional impairments or very high support needs. These homes are built to NDIS design standards for accessibility and support delivery.

We offer a range of SDA models including Fully Accessible, High Physical Support, Improved Liveability, and Robust category homes, in both shared and single-occupancy formats.

Not always. SDA refers to the physical property, while SIL is the daily support provided within the property. Some participants may receive one or both, depending on their assessed needs.

Our Day Programs provide structured activities focused on life skills development, social engagement, health and wellbeing, and recreational learning in a safe and inclusive environment.

We offer cooking classes, art and crafts, community outings, fitness sessions, gardening, technology skills, and personal development workshops—tailored to participant interests and goals.

Participants with funding for group-based community or centre-based activities in their NDIS plan are eligible. We tailor programs based on support needs and age groups.

Respite Care is short-term accommodation and support that gives carers a break while ensuring participants receive quality care in a safe, supportive setting. It’s available for planned or emergency situations.

Respite can be provided for a few hours, overnight stays, weekends, or several weeks depending on your NDIS funding and personal needs.

Yes. We have trained staff and registered nurses who provide high intensity support for participants with complex health, mobility, or behaviour support needs during respite stays.

This service supports participants who require more complex, clinical, or frequent assistance with tasks like PEG feeding, catheter care, bowel management, tracheostomy care, complex mobility support, and medication administration.

Yes, our support workers and nurses are highly trained and qualified in delivering high intensity supports and follow clinical protocols under RN supervision.

We provide high-intensity care both in supported living environments and in your own home, depending on your preferences and support plan.
